LAC+USC Primary Care is actively recruiting a Senior Physician interested in an Associate Primary Care Director position.

LAC+USC Medical Center is a 600 bed hospital that includes a Level-One trauma center and serves over 10 million residents. It is a county affiliated teaching hospital that is in affiliation with the University of Southern California (USC) Medical School in 1885 and has over 900 physicians in their Graduate Medical Education program.

The hospital is in the Boyle Heights neighborhood of Los Angeles. It has over 100,000 residents and has a majority Latino population. It is located east of the Los Angeles River and its neighboring communities include Lincoln Heights, Chinatown, Commerce and Downtown Los Angeles.

Position Summary:

The Associate Primary Care Director at LAC+USC Medical Center is responsible for supporting Patient-Centered Medical Homes (PCMH) as the model for primary care delivery at LAC+USC Medical Center, covering both attending-based and resident-based practices.  In this capacity, the Associate Primary Care Director is responsible for guiding and overseeing elements of PCMH implementation, including empanelment and panel management, adoption of a team-based model of care, population health management, and implementation of care management protocols and integrated behavioral health.  The Associate Primary Care Director will work with the Primary Care Director and Primary Care Nurse Director to develop and implement an optimal process for managing demand for primary care services to optimize care coordination and care continuity across all PCMH clinics.

In this capacity, the Associate Primary Care Director will report functionally to the Director of Primary Care at LAC+USC and will work in collaboration with other Medical Directors, Nurse Managers and Administrators within LAC+USC to ensure primary care is delivered in a cost-effective manner and according to recognized standards for quality, productivity, and access.

Requirements: 
Meets all qualifications for a DHS Senior Physician.

  1. Prior experience in leadership under the patient-centered medical home model.
  2. Prior experience in leadership advancing the primary care integrated behavioral health model.
  3. Prior experience in leadership addressing social and behavioral determinants of health.
  4. Prior experience in leadership utilizing quality improvement methodology to improve clinical operational metrics and clinical quality metrics.
  5. Prior experience in leadership addressing pay-for-performance quality metric programs (i.e., PRIME, QIP).
