Urgent Care / Primary Care Physician Specialist

San Gabriel Valley Health Center Group (SGVHCG) at El Monte Comprehensive Health Center

POSITION DESCRIPTION:

This position affords the physician an opportunity to work in different venues within the El Monte Comprehensive Health Center. Physicians will be able to spend part of their week working in primary care.
SGVHCG uses a patient-centered medical home (PCMH) model wherein patients are provided services by the same team of healthcare providers to support their health needs. Physicians will also provide care to DHS patients in EMCHC’s recently opened and newly renovated urgent care clinic. Onsite, there is laboratory, pharmacy, radiology, and care management support. Our PCMHs are also supported by a robust behavioral health team.

QUALIFICATIONS:

Must be a licensed medical doctor. Prefer physicians who are residency trained and have successfully completed residency in family medicine, internal medicine, or internal medicine/pediatrics. Board certified or eligible. All board-certified physicians receive an annual bonus of 5.5% of their base salary.

PRIMARY CARE RESPONSIBILITES:

• Obtains and performs thorough and precise histories and physical examinations on patients at a frequency determined to be appropriate, by service.
• Applies current medical knowledge when diagnosing and treating patients.  Follows clinical guidelines where appropriate.
• Develops a plan of care, where appropriate.  Schedules patients for follow-up care as indicated.
• Performs approved clinic procedures, for which he/she has current privileges, with skill and expertise.
• Provides appropriate patient and family education regarding the patient’s diagnosis, treatment, diet, medical equipment and/or additional discharge instructions when indicated.
• Follows local (e.g., L.A. County DHS Expected Practices) and/or national guidelines in practicing preventive medicine, as appropriate.
• Provides appropriate patient and family education regarding health maintenance and preventive medicine, as appropriate.
• Recognizes the importance of psychosocial / behavioral medicine in the care of patients.  Treats such conditions or refers appropriately.

RESOURCE UTILIZATION:

• Orders diagnostic tests judiciously and as clinically indicated. Reviews and signs all test results in a timely manner and takes appropriate action.
• Appropriately refers patients externally for emergency care or admissions as clinically appropriate.
• Appropriately refers patients internally for specialty care, social work or other available services.  Follows referral guidelines, where available.
• Prescribes formulary medications whenever possible.
• Prescribes more moderately priced medications when available, as clinically indicated.

URGENT CARE RESPONSIBILITES:

• Evaluate patients presenting for unscheduled and on-demand care in the Los Angeles County Department of Health Services urgent care clinics. Patients of all ages are seen in our urgent care clinics and providers trained in evaluating all ages are preferred.
• Work up urgent/emergent medical conditions including chest pain, abdominal pain, headaches, respiratory illnesses, orthopedic injuries, and other presentations.
• Perform basic procedures including: abscess incision and drainage, suture and staple removal, hernia reduction, closed fracture reduction and splinting, closed joint dislocation reduction, removal of foreign bodies embedded superficially in skin, Bartholin’s gland cyst incision and drainage, laceration repair, felon/paronychia drainage, joint aspirations and injections, epistaxis control measures, low risk nerve blocks, bladder catheterization, superficial hemorrhage control (e.g. bleeding varicose vein), trephination of subungual hematoma, needling and trigger point injections, application of sealant to fractured teeth, ingrown toe nail removal, IUD placement and removal, and other procedures providers may feel comfortable performing.
• Address routine health maintenance needs of patients empaneled to the Los Angeles Department of Health Services, such as administering vaccinations and ensuring appropriate cancer screening.
• Provide continuity care when patients have not been able to access their primary care doctors including refilling and titrating medications and managing chronic health conditions including hypertension, diabetes, heart failure, COPD, asthma, and other health conditions routinely managed by primary care physicians.
• Coordinate referrals for patients to specialty care or other ancillary services including social work, behavioral health, health education, medicolegal services, substance use disorder treatment, and other wrap around services to comprehensively address patient health needs.
• Accurately identify patients who require a higher level of care and assist in organizing transfer of these patients to appropriate care environments, including communicating with EMS staff and/or emergency department staff to facilitate transfers.
• Assess patients for psychiatric emergencies, including identifying when a patient may be a possible danger to themselves, others, or gravely disabled requiring psychiatric evaluation to place the patient on an involuntary hold.
• Full-time physicians are also encouraged to help participate in quality improvement and system improvement work to maximize the efficiency, value, impact, and positive patient experience of the care we deliver.

FACILITY SUMMARY:

The San Gabriel Valley Health Center Group includes El Monte Comprehensive Health Center (EMCHC) and La Puente Health Center (LPHC) and is part of DHS’ Ambulatory Care Network. San Gabriel Health Center provides urgent care, adult and pediatric primary care, women’s health, and specialty care. Together, SGVHCG provides care for a total empanelment of around 52,000 patients. It is located in the city of El Monte, about 12 miles east of Downtown, Los Angeles, in the San Gabriel Valley. The city has an estimated population of 120,000 residents. The city was founded in 1850 and is near Interstate Freeways 10and 605. EMCHC is walking distance from major public transportation, including a MetroLink train station.
