POSITION SUMMARY:
The Patient Safety Officer (PSO) is a faculty physician who is responsible for developing and directing the Harbor-UCLA Medical Center Patient Safety Program and representing the medical center in the Department of Health Services (DHS) Patient Safety Program. The PSO works collaboratively with hospital leadership to establish a culture of safety and identify opportunities and initiatives to enhance patient safety. This position has broad responsibility for promoting a safe and just culture of improvement, supporting health equity, and educating the workforce at all levels about safe patient care. The position investigates patient safety adverse events, develops Plans of Corrective Action (PoCAs), and oversees their implementation. Their focus is to improve care systems and prevent medical errors by promoting continuous improvement and ensuring compliance with applicable regulations.

RESPONSIBILITIES:

  • Collaborate with clinical leaders to develop, implement, and oversee hospital’s patient safety plan
  • Design, modify, and implement care processes to promote patient safety and prevent medical errors
  • Perform Root Cause Analysis (RCA) of adverse events and develop PoCAs based on patient safety principles and guidelines
  • Assure compliance with regulatory requirements related to patient safety, including the National Patient Safety Goals, CMS Conditions of Participation, and the Leapfrog Hospital Safety Grade
  • Direct the Helping Healers Heal and TeamSTEPPS programs, including organizing the program administration and training participants at all levels
  • Co-Chair the Harbor Patient Safety Council; Quality, Risk and Safety Committee; and related workgroups
  • Serve as a member of the Medical Center’s leadership team
  • Provide education to the medical center departments and workforce regarding patient safety activities
  • Represent Harbor on the DHS Patient Safety Committee and participate in the DHS Patient Safety Program
  • Devote approximately 25% of time to clinical care and bedside teaching for residents, fellows, and medical students as a faculty member based on the applicant’s board-certified or board-eligible specialty, including faculty appointment at the David Geffen School of Medicine at UCLA
  • Perform related duties as assigned

DESIRABLE QUALIFICATIONS:

  • Excellent leadership and written and oral communication skills
  • Additional training and/or experience in management, administration, or leadership
  • Experience in patient safety, staff wellness, quality improvement, or related programs
  • Experience in adult learning and instruction
  • Ideal candidates will be show excellent leadership skills and be self-motivated, collaborative, and exhibit ability to work with minimal supervision to meet deadlines in a fast-paced and rapidly changing environment

Harbor-UCLA Medical Center is located in the city of Torrance, a city in the Southwestern (South Bay) region of LA County. Its neighboring cities are Lomita, Redondo Beach, Rolling Hills, and Palos Verdes. The city has over 30 city parks and includes a total of 1.5 miles of beaches on the Pacific Ocean. It has an estimated population of over 145,000 residents.
