Monthly Salary Range: $20,424.00 – $27,383.00

Position: The Ambulatory Care Network (ACN) is currently seeking a highly qualified individual to fill the position of Chief Physician I to implement the strategic direction, policies, and operations for primary care in the ACN. The Chief Physician I will function as the Director of Primary Care, reporting to the Chief Medical Officer (CMO) of the ACN, and will have a dotted-line reporting relationship to the Department of Health Services (DHS) Director of Primary Care. The Health Center Group (HCG) Primary Care Leads and HCG Medical Directors will have a dotted-line reporting relationship to the Director of Primary Care. This position represents ACN on the DHS Primary Care Leadership (PCL) team and works collaboratively with them to ensure a system approach for Primary Care across DHS.

Responsibilities include but are not limited to:
• Works with DHS PCL developing strategies to ensure primary care services are delivered in a patient-centered, cost-effective manner and according to recognized standards of care, including meeting access and availability standards.
• Leads monthly ACN PCL meeting which includes HCG Primary Care Leads and nursing and behavioral health representatives to ensure clear and consistent communication with the DHS PCL team.
• Sets operational expectations using standards and metrics with DHS PCL and works with ACN PCL and ACN medical, nursing, Behavioral Health Integration (BHI) and operational leadership to provide support and resources.
• Meets regularly with individual ACN Primary Care Leads to obtain feedback and ensure that operational expectations are being carried out as planned.
• Collaborates with ACN PCL and DHS PCL to explore innovative approaches to primary care services and to continuously refine the DHS Patient-Centered Medical Home (PCMH) model.
• Works with ACN PCL and ACN leadership to develop and implement ongoing training and support for PCMH teams.
• Works with the DHS Director of Primary Care Clinical Quality and the ACN CMO to ensure that ACN meets clinical benchmarks, specifically related to the National Committee for Quality Assurance (NCQA), Quality Incentive Pool (QIP), California Advancing and Innovating Medi-Cal (Cal AIM), Healthcare Effectiveness Data and Information Set (HEDIS) and any value-based reimbursement initiatives.
• Works closely with ACN Performance Improvement to identify opportunities for Performance Improvement (PI) work and to support teams in the implementation of PI efforts.
• Works with DHS and ACN stakeholders to ensure that transitions to/from primary care in the ACN are seamless, effective, and non-duplicative.
• Serves as the primary point person in the ACN for collaboration with key DHS workgroups. These include, but are not limited to specialty care workgroups, the DHS HIV Best Practices Workgroup and the DHS Pediatric Primary Care Group.
• Collaborates with stakeholders to develop strategies supporting equity in health care, and towards identifying and addressing health disparities.
• Works with ACN PCL team to support recruitment and retention of a primary care provider workforce that meets the needs of the patient population.
• Provides direct patient care at an ACN facility 10% of their total time.

Desirable Qualifications:
• Experience and comfort using data to drive transformation.
• Experience in a Labor Management Partnership environment.
• Experience as a primary care physician in Family Medicine or Internal Medicine preferred.
• Experience working as a physician in a patient centered medical home environment.
• Experience leading a cross-disciplinary team.
• Experience working in government or non-profit environment serving a diverse medically underserved population.
• Excellent written and verbal communication skills.
• Creative, forward-looking thinker.
• Ability to work collaboratively and effectively with other members of the leadership team.

Job Appointment Options:
• To be considered as a lateral transfer, candidates must be a current permanent county employee who holds the payroll title of Chief Physician I – Family Practice or Chief Physician I – Internal Medicine.
• To be considered as a new hire, candidates must be reachable on the DHS certification list.
