Adult Primary Care Assistant Clinic Medical Director

Services are provided in a safe, patient-centered environment through a network of 8 medical home clinics that deliver primary care for both adults and children. Los Angeles General Medical Center practices in a patient-centered medical home model and patients are provided services by the same team of healthcare providers to support all their health needs. The medical home serves as a hub for the larger network of hospitals, emergency departments, urgent care centers, behavioral health providers, and specialists within the Los Angeles County Department of Health Services.

Los Angeles General Medical Center is seeking a qualified adult primary care clinic assistant medical director (BC/BE in Internal Medicine, Family Medicine, and Medicine+Pediatrics). The PCMH Assistant Medical Director at Los Angeles General Medical Center is responsible for establishing and maintaining a Patient Centered Medical Home (PCMH) as the model for primary care delivery for their clinic, covering a high-performing and nationally recognized resident-based practice.

In this capacity, the Assistant Medical Director is responsible for supporting all elements of PCMH implementation, including direct patient care, resident supervision and didactics, empanelment and panel management, adoption of a team-based model of care, population health management, and implementation of complex care management and integrated behavioral health.

The Assistant Medical Director will work with the PCMH Clinic Medical Director and Clinic Nurse Manager to develop and implement an optimal process for managing demand for primary care services in the PCMH and to optimize care coordination and care continuity. In this capacity, the Assistant Medical Director will report functionally to the Clinic Medical Director and will work in collaboration with other Medical Directors, Nurse Managers and Administrators within Los Angeles General Medical Center to ensure primary care is delivered in a cost-effective manner and according to recognized standards for quality, productivity, and access.

The hospital is in the Boyle Heights neighborhood of Los Angeles. It has over 100,000 residents and has a majority Latino population. It is located east of the Los Angeles River and its neighboring communities include Lincoln Heights, Chinatown, East Los Angeles and Downtown Los Angeles.
